Leeds and the Thousand Islands residents and organizations may request congratulatory certificates from the Mayor and Council for anniversaries, birthdays, special honours, and business achievements.
Requests must be submitted a minimum of two weeks prior to the event. Certificates are issued at the discretion of the Clerk.
If you wish to request the Mayor or a member of Council to present the certificate at the event, please include these details in the "other details" section of the form. These request are subject to the Clerk's discretion and Mayor and Councillor availability.
